Does giving an antibiotic IV push affect the pharmacodynamic exposure?
This question was examined for meropenem, cefepime, and aztreonam in a study published in American Journal of Health-System Pharmacy May 2017. In this study, previously published pharmacokinetic data for meropenem, cefepime, and aztreonam was used. The authors performed Monte Carlo simulations of 30-minute and 5-minute infusions to assess the probability of attaining appropriate time above MIC for each antibiotic. The authors noted no to minor differences between 30-minute and 5-minute infusions for most dosing regimens. Therefore there appears to be no concern from a pharmacodynamic standpoint of giving these antibiotics IV push.
